Not so much “problem” as “potential story hooks”, I’d say. Werewolves aren’t good with people. Cities have lots of people.
As for the spiders and rats, these are the Forsaken’s ancestral enemies. They are pretty much everywhere, including the cities. Werewolves probably won’t have a choice but to encounter them and/or to be forced to do something about them.

This is how Werewolf can be played, yes. There are many sources of stress and frustration for the Forsaken, and when caught at a bad time, there’s always the very real chance of snapping and hurting bystanders. Plus, human flesh is addictive and provides very real and important benefits to werewolves.
Interactions with humans are interesting. You need them to stay sane, but at the same time you don’t want to get too close, for fear of getting them hurt.
